ICBCH Certification Graduates Submission

Instructions:


Step 1:

This is the official page to submit your request for ICBCH Membership Certificates and ICBCH Certification Certificates. Please complete this form with the following information for each student:

The student's full name exactly as they wish it to appear on the certificate
The student's graduation date, which will be the date printed on the certificate
The student's email address
The student's level of certification, selected from the options provided. We offer the Certified Hypnotist (CH) title at 40 hours of training, and the Certified Professional Hypnotist (CPH) at 100 hours.

Please ensure all information is accurate before submitting, as it will be used to generate the official certificates.


Step 2:

We will invoice you at your email address for $47 per graduate.


Step 3:

After payment of your invoice, we will then send you PDF copies of their individual certificates, membership certificate, and an instruction sheet so they can self-register for ongoing membership renewals. Their first year of membership will already have been paid for by you.


Step 4:

You will distribute the certificates to your new graduates via email or hard copies created at your location, either by printing them at a local printshop or on your own printer. We do not mail any physical certificates from the office, nor do we email directly to your graduates.
